Chucky got his stitches after his eventual ex girlfriend and wife Tiffany added them.
It stemmed from a scythe that cut him up in Child’s Play 3 at the conclusion of the film.
It’s an iconic look that took his original image to the next level.
He was stitch free up until Bride of Chucky where it stayed but went away again once the TV series Chucky debuted.
The look with his stitches extended past his face all the way up to even his hair to show how bad his injuries really were.
An inconsistency with this look that I noticed was in Curse of Chucky where it didn’t show until near the end but it didn’t cover his hair.

Chucky in his universe was the popular children’s toy of the 80s.

It’s a bright red haired doll that seems equipped with military and construction equipment for accessories.
It’s fully iconic to the point where the color scheme loudly communicates the relation to Chucky.

Chucky began having a face full of stitches at the start of Bride of Chucky.
Thanks to his girlfriend at the time, Tiffany, she gathered his remaining parts, sewed them up and performed his voodoo chant.
It was this movie that was the start, and since then he kept his stitched up face until the beginning of Cult of Chucky.
We learn he splits up his soul, and Andy kept his severed head, most likely from the end of Seed.
But after Chucky splits his soul up, it’s put into other dolls that were finished without blemishes.
It also continues into Season 1 of Chucky as well.
